Cardiovascular-renal-metabolic risk: early identification by the general practitioner

Abstract
The large link of pathophysiological interactions between obesity, type 2 diabetes mellitus (DM), cardiovascular disease (CVD), and chronic kidney disease (CKD) has been defined as cardiovascular-renal-metabolic syndrome (CKM) by the American Heart Association (AHA).
This review examines pathophysiological mechanisms, risk factors and their management in general practice.
In this context it’s easier to promote prevention and therapy in a way that is personalized and adjustable to the social context which patients belong: that is significant because the great potential of improvement in patient with CKM.
This review reports useful approaches regarding assistance to screening, patient education, and initial therapy of CKM. It is also highlighted when it’s imperative the interdisciplinary coordination of care with the specialist physician to prevent adverse clinical outcomes, in particular
